Essay on Comprehensive Health Assessment

Comprehensive health assessment
Comprehensive health assessment programs are plans that are used to achieve improved health of individuals in the community so that they can have better outcomes in physical, psychological and work life. The goal of these programs is to assist individual to have a vigorous life. In these programs the authorities that developed it do the analysis to find out the individual’s degree of weakness in the certain aspect of health. The person’s social and personal problems and history of his illness, all are considered in this program so that the nurse can guide him by using more accurate piece of information. Physical activities have a great contribution in the state of person’s physical condition. Activities that provide fitness are promoted where as activities like late sleep, smoking or eating junk food is discouraged for living a healthy life. Health assessment is mostly implemented in a way that is non medical or informal. By these agendas health is promoted in the common public. These programs help inhabitants to know their weak areas, and they required to give priority to these critical areas of health so that they can improve their life. Those who have adopted a life style that has bad affects on their body are discouraged in the assessment programs. A program that is developed by using better strategies may be broader in scope, it helps and guides people by not only taking their health and lifestyle in consideration but also takes psychological and cultural areas of individual’s life in account in order to provide a comprehensive assessment [David Haber, 2010].
A nurse health assessment is a program in which she collects data systematically and analyzes the composed data to find out the degree of healthiness and the correctness of life style of a person, family or the whole community. She collects data and formulates it to make diagnosis. These analyses help her in finding the critical areas which require improvement. Collaborative problems are also identified in order to help the community to have a mutual healthy life and environment to live within. She identifies the strength of a person in having a better lifestyle. Health assessment agendas are not limited to physical body strength; inspections of community’s life style are also a part of the plan. The person’s functionality is measured by collecting and analyzing physiological, spiritual, socioeconomic and cultural variables. The culture is also involves in the health system of a person as it is involved in the health system of a community. Psychological and socioeconomic, impacts the mental health and hence it is necessary to examine this area to find out the health status. This helps the individual to understand his body’s requirement for development [Kay Saucier Lundy, Karen Saucier Lundy, Sharyn Janes, 2009].
Structure of the program:
The health assessment programs in many organizations are developed on ad hoc bases. These are not a long term programs, they are established by the trigger from a particular event and ends up by the end of that event. The formal structure of these programs should be comprises of these integrated elements; Goals and objective should be set clearly before the achievement of program by the authority, these goals should be attainable and acceptable by the employees. The authority should be involved in the process explicitly, they allocate resources and they should be cleared and active to achieve the desired goal and objective. The employees should be placed appropriately and there should be mutual understanding of the employees to have an effective coordination. To achieve a better understanding of employees, different programs of communication should take place within the department under mid level managers and employees. It is observed that many organizations have developed committees that are set to provide communication between employees of different backgrounds on political bases. A person should be assigned at the post of program director that has the ability to administrate the assessment program. This person should be trained and experience in the requisite field. A feedback should be taken in order to confirm the validity of the program by participants and non participants if possible. Feedback will be helpful in finding the lack areas that require enhancement; it also serves as a tester to find the popularity of the program among the people of a particular community. These programs should contain the procedures that are applied to maintain the confidentiality of the person’s information. A record keeping mechanism is used to keep track of the activities so that the monitoring and evaluation should be take place more appropriately. The analysis and compilation is than take place to report the success of the program to the manager. This evaluation will be helpful to make changes in the program if required [Jeanne Mager Stellman, International Labour Office, 1998].
There are myriad activities that take place in the event. These activities can be continuous or can be addressed periodically. In this event individual, community or both individual and community can be targeted for the health assessment. The major objective is to provide information about how to prevent oneself from diseases or from conditions that can cause disabilities in them. Clinical services are the first one in the list of activities that are achieved in the health assessment program. The nurses are required in this activity to examine the body of an individual medically, she takes screening tests and the patient go throw diagnosis actions. These actions may include mammography in which the breasts are examined to detect tumor, pap smears; it is a test for the diagnosis of cancer. In this test a smear of exfoliated cells are examined by using microscope. Other tests include a test to check cholesterol level, immunity etc. The nurse also motivates the participants to modify their habits in order to improve fitness and weight control. The participants are discouraged for their habit of smoking by the employees of health assessment program. In the activity of health education, nurses are trained to promote awareness about diseases especially about the epidemic diseases and describe various methods or prevention. The people are given information about the importance of healthy life, the methods to reduce risk factors. The healthy lifestyle can only be maintained by ceasing unfit habits. In this activity the people are motivated to take advance actions for healthy living because prevention is always better than cure. In the next activity people are given guidance to manage medical care. In this session experts give advice to the people about the management of diseases like when to go for work after illness, how to control drugs usage and how to deal with the drugs addicts and how to behave morally in a chronic disease and the patient of chronic disease and so on. This is not the limit, in the health assessment programs the individuals are also educated to learn how to cop up with their personal problems like how to manage their depression, how to plan for retirement. Information and help are also provided for those who need them in family related issues like family planning, care of parents, care of family members and so on. Activities involving assessment in professional health are also provided. The whole process of health assessment is very helpful in gaining a healthy life and environment [Jeanne Mager Stellman, International Labour Office, 1998].
Nurse strategies:

A nurse plays a very important role in the comprehensive health assessment program because she collects the data about the physical, psychological and cultural state of the person and evaluates it to make diagnosis. In the program the person and his family both are involved throughout the process for surety of correctness of information. The nurses that are involved in this program are professionally trained for the assessment. The first strategy of the nurse is to collect comprehensive information of the patient according to the situation by using appropriate methods. She made diagnosis by evaluating and analyzing the data collected from the patient [Frances A. Maurer, Claudia M. Smith, 2000]. She identifies the outcome from cure plan using the set of information from the assessment. This is done by the interdisciplinary team assigned for the health care. They develop plans to provide solutions to health related issues of people or community and also develop alternatives if the stated plans failed. This development of alternative improves the authentication of procedures that provide better solution. These activities are adopted because the main strategy of the nurse is to provide solution to the problems related to health and to attain a better lifestyle in the community. The fifth strategy of the nurses is the implementation of the plan. She implements the developed program by all possible means and encourages her patients to adopt the solution provided by her for their health problems. She coordinates them in the process of treatment by care. The nurses are trained to promote safe and vigorous environment by the health assessment programs. They consult people to enhance their abilities and to lead them to accept the plans and implement it in their life. She encourages those who have a bad life style to bring a positive change in their life. In treatment nurses use methods, therapies, and referrals that are prescribed by the federal laws and regulations. After the adaptation of treatment their strategy is to evaluate the healing process so that she can assure the successful outcome [Marianne Matzo, Deborah Witt Sherman, 2010].

The world health organization describes a healthy person as an individual who is in a social, mental and physical good state. The identification of a person’s healthy status is sometimes very complex because physical fitness can be identified by oneself but social or mental disturbance are not easily acknowledged [World Health Organization, 1999]. Health promotion programs were initiated to acknowledge the well being of a person in his environment. The people should be able to function well in their daily life. The main objective of the program is to achieve communities in which an individual serves as healthy part of the society. In order to promote health system, nurses are trained to provide assessment to the individuals. They guide them in their physical, mental or families related problems. Several organizations establishes programs for health assessment, the structure of these programs is based on some integral elements. These elements include statement of goals to achieve by this program; the promotion of mutual understanding of employees to achieve the task; the selection of program director with appropriate skills; implementation of feedback mechanism; mechanism to ensure the confidentiality of the individuals; recording of activities and compilation of these events. In the event of health assessment, the activities that are desired to achieve are the clinical services, health related education, guidance and solutions. Nurses that are involved in the health assessment program are highly trained. Their main objective is to provide guidance to have a healthy life and information to prevent them from illness or disabilities [Carl I. Fertman, Diane D. Allensworth, 2010.].
